How to know if you have an STD? Rape?

I was raped by a neighbor, and well, I have kicked and screamed my way out of all my medical exams. I have a lot of mental disorders, and being looked at or touched down there, even by a professional, freaks me out. I can't do it! But I also know that I need to be watching for signs of STDs/HIV. By rapist slept around a lot, it wouldn't be surprising if he carried a disease. Also, how can you visibly tell if your hymen is broken? I know it probably is, that is what rape does, but in a sense, seeing visibly that it is broken is my way of closure, of admitting to myself this actually happened, and then getting help for it.
